ROADMAP for Lifestyle CHANGE


The Five Seasons Health Pathway for Change process embodies a specific set of activities beginning with exploration and ending at the completion point. These activities are: Identify, explore , validate, refine, prioritize objectives and set SMART goal s, define plan , define measures for return on coaching investment (R0I) , create steps, milestones, and accountability framework, work the plan , gather and discuss results, calculate return on coaching investment (ROI), wrap-up , and close-out engagement.


The length of time devoted to creating change will be individual-specific and proceeds with your level of readiness.  As an average, significant results can be achieved in as little as three months.


Throughout the process, you are respected as the expert with the ability to determine your best, personal wellbeing pathway. In healthcare terms, you are considered the “single source of truth” about yourself. The coach’s role is to facilitate client insight, prompt for deeper introspection and clarity for value and direction of lifestyle change.


Your coach can also direct you to information, knowledge, and additional resources to support your wellbeing goals.  If needs arise which are out of scope for the coaching engagement your coach can provide references and help you find the right resources.  If you are adding a health coach to your existing health care team, the coach’s role is to support you in understanding, navigating, and following your teams’ recommendations.
